Propose written decision records

Claim qualified to a four-week trial. Warrant made explicit: our rework comes from divergent understanding, and writing decisions down is its cheapest fix. Counterargument — "bureaucracy" — pre-empted: two minutes per decision, stop if it doesn’t help.

After feedback: reframed from criticism of past mistakes to a system fix. Separate the people from the problem.
